WALKER SPORTS ACADEMY PARTICIPATION WAIVER

I am the parent or legal guardian of the player named in this registration, or I am the player and I am 18 or older. I want that player to take part in training, camps, and private lessons with Walker Sports Academy.

I understand football, quarterback training, and speed and agility work include running, throwing, cutting, and other physical activity that can cause injury, including serious injury. I accept those risks for this player. The player is physically able to participate, and I will tell the coaches about any condition that affects participation.

If I cannot be reached, I give Walker Sports Academy staff permission to arrange emergency medical care for the player. I am responsible for the cost of that care.

I understand the price shown before payment. Camps and private lessons are a one-time charge. A group session is a one-time drop-in unless this email has an active monthly plan for that program with sessions left in the current billing period. A monthly plan includes a set number of group sessions. Unused sessions expire at the end of that billing period and do not roll over. One registration covers one player. A sibling is a separate signup. This site does not cancel a single session or a single registration. The academy can pause or cancel a monthly plan from the admin login.

I allow Walker Sports Academy to use photos and video of the player from training for academy purposes.

The name I enter is my signature. I agree to this waiver before any payment is taken.
